This case report describes a rare presentation of massive ascites and right-sided pleural effusion resulting from intra-abdominal endometriosis, noting that fewer than ten such cases have been documented globally. The authors emphasize that the final diagnosis was significantly delayed, highlighting the diagnostic challenges associated with this uncommon manifestation of the disease. They recommend that clinicians maintain awareness of endometriosis as a potential cause for these symptoms after ruling out more common etiologies like malignancy or tuberculosis. This paper is centrally about endometriosis — specifically focusing on its rare extra-pelvic complications involving pleural fluid accumulation and ascites.
