This chapter discusses operative treatment strategies for inflammatory adnexal diseases, focusing mainly on tubo-ovarian and pelvic abscess management, drawing on literature largely from 1975–1994. It outlines a range of approaches from conservative broad-spectrum antibiotic therapy to immediate laparotomy, noting that many authors recommend antibiotics first with surgery reserved for treatment failure or abscesses larger than 8 cm, and reports antibiotic-only success rates varying widely (16–81%). The chapter states that laparoscopy has been the preferred surgical access for over 15 years, with recommendations for intervention within 24 hours after hospital admission or antibiotic initiation, but emphasizes that no prospective randomized studies compare approaches, and that differences in surgical radicality and antibiotic regimens complicate comparisons of success rates, complications, and postoperative pregnancy rates. The paper does not explicitly discuss endometriosis or adenomyosis; it was included in the corpus via a keyword match in the upstream search index.
