Factors associated with a poor prognosis for the IVF-ICSI live birth rate in women with rAFS stage III and IV endometriosis
This study identified active smoking, poor ovarian response, and rAFS stage IV endometriosis as factors associated with a decreased cumulative live birth rate in women undergoing IVF-ICSI.

This retrospective cohort study (2010–2014) analyzed 101 women with rAFS stage III and IV endometriosis who underwent 232 IVF-ICSI cycles and 212 embryo transfers, assessing factors associated with lower cumulative IVF-ICSI live birth rates per cycle and per patient. Cumulative live birth was 14.7% per cycle and 31.7% per patient, and it was significantly decreased by active smoking, poor ovarian response defined by the Bologna criteria, and having rAFS stage IV disease (adjusted odds ratios reported in the abstract). The cumulative live birth rate was 59.4% when no poor-prognosis factors were present, 25.6% with one factor, and 7.7% with two or three factors, indicating a stepwise reduction. The paper’s limitation is that it is retrospective in design, and it concludes—within its data—that smoking and ovarian response status negatively affect cumulative live birth in this population. This paper is centrally about endometriosis—specifically identifying prognostic factors for IVF-ICSI cumulative live birth in rAFS stage III/IV endometriosis.
