Abstract

症例は24歳，女性．23歳時に人工妊娠中絶の既往があり，2002年10月初旬より血痰と喀血が出現した．その後，毎回の月経開始前後に血痰を認めるようになった．血痰出現時に撮影した胸部CTでは左S3，右S8，右S6に浸潤影を認めた．気管支鏡検査では左B3aからの出血を認めたが，経気管支肺生検では異所性子宮内膜組織は得られなかったため3ヵ月間経過観察されたが，症状が続くため胸腔鏡下手術を施行した．肉眼的に胸膜直下の出血として病変部位を同定することができ，病理組織学的に異所性子宮内膜を証明し得た．胸腔鏡下手術は診断と治療に有用な方法の1つであり，確実な切除のためには月経時期にあわせた胸部CTおよび気管支鏡検査による出血部位の同定が重要である．
