Inguinal Endometriosis: An Uncommon Differential Diagnosis as an Inguinal Tumour

This case report describes a 48-year-old woman with a tender, relatively fixed right inguinal mass and menometrorrhagia, with history of groin pain during menses, in whom fine-needle aspiration biopsy of the mass showed endometriosis. Surgical excision removed a solid fibroid-like tumor, and histopathology confirmed endometrial-type glands and stroma; uterine curettage showed disordered proliferating endometrium, and the patient remained asymptomatic after simple excision. The paper reviews that inguinal endometriosis is rare (only a few dozen cases reported), is frequently right-sided, and is often confused with other inguinal tumors or hernia-related conditions; it also notes diagnostic uncertainty because imaging is not specific and preoperative diagnosis is made in less than half of cases, with malignant transformation reported in some literature.
