Abstract

primary adenocarcinoma of the vagina arising in endometriosis is extremely rare. We present here the fifth case in the literature and the only one in which long-term follow-up is available. The patient was managed with intravaginal and external beam irradiation therapy and remains alive and free of disease 7 years after the completion of the treatment. The literature is reviewed with reference to the incidence, histopathology, and treatment of vaginal malignancies arising in endometriosis. Since the initial description of ovarian carcinoma developing in endometriosis by Sampson in 1925 [ 11, occasional case reports have appeared documenting the development of sarcomatous and carcinomatous malignancies in pelvic and extragonadal sites of endometriosis. These reports have been reviewed by Scully er al. [2], Brooks and Wheeler [3], and most recently by Mostoufizadeh and Scully [4]. Of the 37 cases of extraovarian adenocarcinomas arising in endometriosis, 14 were reportedly located in the rectovaginal septum and only four adenocarcinomas were noted to have arisen in vaginal endometriosis. These 4 patients were treated with surgery alone and only short follow-ups were provided. This communication presents a fifth patient with adenocarcinoma arising in vaginal endometriosis who remains free of disease for over 7 years following management with intravaginal radium applications and external beam pelvic radiation therapy.
