This retrospective study correlated preoperative serum Ca-125 levels with laparoscopic findings in 350 women with pelvic pain symptoms suggestive of endometriosis (Jan 2010–Mar 2013), comparing Ca-125 35 U/mL. Women with ovarian endometriomas were then excluded for a secondary analysis, and both analyses compared the presence of endometrioma, deep infiltrating endometriosis (DIE) lesions (including intestinal, bladder, and ureteral sites), lesion counts, and the American Society for Reproductive Medicine (ASRM) score. Higher Ca-125 (>35 U/mL) was associated with more frequent endometrioma, higher rates of DIE and intestinal DIE, and higher ASRM scores; similar associations persisted after excluding ovarian endometrioma patients. The paper’s main limitation is its retrospective design, and it specifically notes results derived from a laparoscopic-treated cohort without additional stated external validation.
