Concordance et apports de l’hystérosalpingographie et de la cœlioscopie dans l’exploration tubaire et pelvienne en cas d’infertilité

This prospective study in 120 infertile patients compared hysterosalpingography (HSG) with laparoscopy findings for tubal and pelvic evaluation, with tubal patency on laparoscopy confirmed using a methylene blue test; the key limitation explicitly reflected in the design is that the examinations can be separated by up to 3 months and HSG performance can be affected by technical/anesthesia factors. Concordance between HSG and laparoscopy for tubal obstruction was moderate, with Kappa values of 0.42 overall (0.48 proximal, 0.53 distal), and discrepancies included cases where HSG suggested patency but laparoscopy judged tubes occluded, alongside some cases of proximal occlusion despite normal HSG. Laparoscopy additionally detected pelvic endometriosis (7%), pelvic adhesions (33%), and tubes that were patent but had pathologic appearance (20%) that were not suspected on HSG, while the reported sensitivity of HSG for detecting pelvic adhesions was low (9.5%). Relevance to endometriosis: endometriosis was directly identified by laparoscopy in 7% of patients and was among the conditions laparoscopy could add beyond HSG, though the paper’s main focus is comparing diagnostic concordance between HSG and laparoscopy in infertility rather than endometriosis mechanisms.
