O artigo relata o caso de uma paciente de 39 anos com dismenorreia intensa desde a adolescência, sintomas gastrointestinais e diagnóstico previamente incorreto, cujo diagnóstico de endometriose profunda foi confirmado por imagem com múltiplos focos e distorções anatômicas. Após falha de tratamento convencional e uso prolongado de opioides, a paciente iniciou em junho de 2024 óleo medicinal de Cannabis sativa full spectrum contendo THC (15 mg/mL) e CBD (5 mg/mL), titulado até cinco gotas noturnas. Houve melhora significativa referida da dor, do sono, da dispareunia, do humor e do trânsito intestinal, com normalização do sono e melhora global da qualidade de vida nos instrumentos WHOQOL-BREF e PSQI-PT; o uso foi suspenso no pós-operatório por desejo gestacional.
