und Methode Narrative Übersichtsarbeit auf Basis aktueller Leitlinien, Übersichtsarbeiten und klinischer Literatur zu Endometriose-assoziierten Schmerzen, chronischer Schmerzmedizin sowie interdisziplinärer multimodaler Schmerztherapie. Ergebnisse Die Behandlung Endometriose-assoziierter Schmerzen sollte frühzeitig multimodal erfolgen und neben hormoneller sowie symptomatischer medikamentöser Therapie auch edukative, physiotherapeutische, bewegungstherapeutische und psychotherapeutische Verfahren umfassen. Entscheidend ist ein biopsychosoziales Krankheitsverständnis mit Fokus auf Funktionsverbesserung, Selbstwirksamkeit und Vermeidung weiterer Schmerzchronifizierung. Bei chronischen Schmerzstörungen kann eine interdisziplinäre multimodale Schmerztherapie indiziert sein. Diese basiert auf einer eng abgestimmten Zusammenarbeit verschiedener Fachdisziplinen und kombiniert aktive therapeutische Verfahren innerhalb eines strukturierten Behandlungskonzepts. Schlussfolgerung Endometriose-assoziierte Schmerzen stellen häufig ein komplexes chronisches Schmerzsyndrom dar, das eine rein somatisch orientierte Behandlung überschreitet. Die Versorgung betroffener Patientinnen erfordert daher ein interdisziplinäres biopsychosoziales Behandlungskonzept mit frühzeitiger Therapie mit multimodalem Ansatz und bei Bedarf spezialisierter interdisziplinärer multimodaler Schmerztherapie.

Abstract

Background Endometriosis is one of the most common chronic diseases affecting women of reproductive age and is frequently associated with chronic pain, functional impairment, and psychosocial burden. The pathophysiology comprises nociceptive, neuropathic, and nociplastic pain mechanisms. Chronic pain in endometriosis results from complex interactions between biological, psychological, and social factors, requiring a biopsychosocial treatment approach.

Objectives

To provide an overview of interdisciplinary multimodal pain management in endometriosis and to discuss specific aspects of treating chronic pain in affected patients.

Methods

Narrative review based on current guidelines, review articles, and clinical literature addressing endometriosis-associated pain, chronic pain medicine, and interdisciplinary multimodal pain therapy.

Results

Management of endometriosis-associated pain should follow an early multimodal approach including hormonal treatment, symptomatic pharmacological therapy, patient education, physiotherapy, exercise therapy, and psychotherapeutic interventions. A biopsychosocial understanding of pain focusing on functional improvement, self-efficacy, and prevention of further pain chronification is essential. In patients with chronic pain disorders, interdisciplinary multimodal pain therapy may be indicated. This treatment concept is based on close collaboration between different disciplines and combines multiple active therapeutic interventions within a structured framework.

Conclusion

Endometriosis-associated pain frequently represents a complex chronic pain disorder that exceeds a purely somatic disease model. Effective treatment therefore requires an interdisciplinary biopsychosocial approach with early multimodal management and, when necessary, specialized interdisciplinary multimodal pain therapy.
