This case report describes cervical endometriosis, a rare condition of endometrial tissue in the cervix, found incidentally in a post-menopausal woman not receiving hormone therapy.

This paper reports a case of cervical endometriosis in a 54-year-old post-menopausal woman who was not receiving hormone therapy, presenting with a suspected large uterine myoma; routine Pap smear showed only reactive cellular changes with inflammation and HPV DNA testing was negative. Imaging led to a laparoscopic-assisted vaginal hysterectomy, and at surgery there was no gross evidence of endometriosis; histopathology found endometrial glands and stroma in the cervical stroma along with endometrial atrophy and a cervical region leiomyoma. The authors review prior literature on cervical endometriosis, noting reported incidence estimates of 1.6–2.4%, that many cases are asymptomatic and often diagnosed incidentally, and that cytologic features can mimic atypical or cancerous glandular lesions, with limitations that preoperative diagnosis remains difficult. This paper is centrally about endometriosis — it describes cervical endometriosis in a post-menopausal patient and reviews how it is detected and misdiagnosed (including in cytology).
