A case of catamenial pneumothorax with confirmed endometrial tissue on the abdominal side of the diaphragm

Abstract
症例は34歳，女性．7ヵ月間に4度，月経期間中に気胸を繰り返したため，胸腔鏡下手術を行った．胸腔内を観察し，横隔膜腱中心に数ヵ所の瘻孔が認められたため，すべての瘻孔を含むように横隔膜部分切除を施行した．術後病理検査では，胸腔側の瘻孔周囲には異所性子宮内膜組織が認められなかったが，腹腔側から出血斑（blueberry spot）が観察され，異所性子宮内膜組織を認めた．術後9ヵ月後に右気胸を再発したため，ホルモン療法を開始し，以後は再発を認めていない．経胸腔アプローチによる手術療法のみでは腹腔側の病変まで観察することができないことが，月経随伴性気胸の手術単独療法での再発率が高い原因の一つの可能性であると考えられた．
