A patient with bilateral thoracic endometriosis pneumothorax and porous diaphragm syndrome who initially presented with left pneumothorax

Abstract

症例は右付属器切除術の既往のある未経妊の42歳女性．HCV感染症治療後の腹部超音波検査で右胸水を認めた．胸水精査のため施行した胸部単純X線写真で偶発的に左気胸（II度）を認めた．胸部単純CTでは両側肺に多発肺囊胞を認めた．診断・治療目的に胸腔鏡下手術を施行し病理検査にて胸腔子宮内膜症の診断となった．その直後から右気胸と右胸水貯留を繰り返した．右胸水改善と同時に多量の血性腹水を認めたため，腹水穿刺時にアミドトリゾ酸を注入し腹腔内造影を施行したところ腹腔内と右胸腔内の交通が確認された．右気胸の手術と共に腹腔鏡下手術を施行し，横隔膜病変を胸腔側と腹腔側から確認し，両側から人工被覆剤にて横隔膜を補強した．右気胸も病理検査にて胸腔内子宮内膜症の診断となった．右横隔膜交通部が明らかな，左気胸を初発とした両側の胸腔子宮内膜症性気胸の1例を経験したので報告する．
