Tranexamic acid for the treatment of heavy menstrual bleeding: efficacy and safety
Tranexamic acid reduces heavy menstrual bleeding by 26%-60%, improving quality of life with few mild adverse effects, and its use is contraindicated in active or high-risk thrombotic disease.

This paper is a narrative review assessing the efficacy and safety of tranexamic acid for heavy menstrual bleeding (HMB), drawing on English-language studies identified via Ovid Medline, PubMed, and Cinahl (search last updated March 25, 2012). Across included trials, tranexamic acid reduced menstrual blood loss by about 26%–60% and was reported as significantly more effective than placebo and compared options including NSAIDs, oral cyclical luteal phase progestins, and oral etamsylate, though the levonorgestrel-releasing intrauterine system reduced menstrual blood loss more than tranexamic acid. The review reports that tranexamic acid improves quality of life with generally few, mainly mild adverse effects, and it states there is no evidence of increased thrombotic events, while active thromboembolic disease is a contraindication and certain thrombosis histories are considered contraindications in the US. This paper does not explicitly discuss endometriosis or adenomyosis; it was included in the corpus via a keyword match in the upstream search index.
