The final diagnosis required an accurate sampling and the aid of immunohistochemical markers.Conclusion(s)Endomyometrioma is an extremely rare entity. None of the previous cases showed cytologic atypias. The finding of atypias, in analogy with endometriosis, suggests the müllerian metaplastic theory.
