Abstract

症例は36歳，女性．卵巣嚢腫に対する手術歴がある．10日前より心窩部痛が出現し，近医で胃炎と診断されたが，症状が悪化し当科を受診した．臍部を中心とした間欠的な強い腹痛があり，冷汗著明であった．腹部超音波検査で心窩部にpseudo kidney signを認め，造影CTにて盲腸が横行結腸まで重積しており，造影効果の乏しい腫瘤が先進部となっていた．腹痛が非常に強かったため，盲腸腫瘍による腸重積の診断で，緊急腹腔鏡下手術とした．盲腸が肝弯曲部まで重積しており，腹腔鏡下にHatchinson手技で整復した．体外操作で腫瘍を確認すると，盲腸に弾性硬の5cm大の粘膜下腫瘍を認め，先進部となっていた．悪性腫瘍が否定できないため，回盲部切除術D3郭清を施行した．術後経過良好で術後7日に退院した．病理診断では，盲腸筋層を主座とする子宮内膜症を認めた．横行結腸まで重積が及ぶ盲腸子宮内膜症は非常に稀であり報告する．
