This paper reviews the evaluation of female pelvic pain in acute and primary care, defining acute pelvic pain as lasting less than 48 hours and chronic pelvic pain as present for at least 6 months, and emphasizing an evidence-based approach using clinical history and physical examination. It surveys broad differential diagnoses across reproductive, genitourinary, gastrointestinal, and musculoskeletal causes, while also touching on specific acute conditions such as ectopic pregnancy and pelvic inflammatory disease and noting diagnostic limitations (e.g., the differential varies with age and pregnancy status). The major caveat is that the section is focused on acute pelvic pain and relies on the accuracy and completeness of history/exam rather than presenting new original comparative data. This paper does not explicitly discuss endometriosis or adenomyosis; it was included in the corpus via a keyword match in the upstream search index.

Abstract

Pelvic pain is a common complaint in both the acute and primary care settings. The differential is broad and is dependent upon the patient’s age and whether or not she is pregnant. Aside from reproductive etiologies, genitourinary, gastrointestinal, and musculoskeletal causes must also be considered [1]. Acute pelvic pain has been defined as pain lasting less than 48 h, whereas chronic pelvic pain has been present for at least 6 months [2]. This section will specifically focus on acute pelvic pain with an emphasis on an evidence-based approach using clinical history and physical examination skills.
