This case report studied a 31-year-old woman with cyclical pain and a progressively enlarging, tender mass at the site of a previous Pfannenstiel incision after two lower segment cesarean sections, where ultrasonography showed an ill-defined vascular lesion in the subcutaneous scar. Based on history, examination, and imaging, the authors diagnosed scar endometriosis and performed wide local excision, with intraoperative findings of a mass encased in fibrosis extending from the subcutaneous plane, infiltrating the rectus sheath, and reaching the anterior surface of the uterus. Histopathology confirmed endometrial glands with stroma and associated fibro-collageneous changes, leading to the final diagnosis. As a single-patient report, the main limitation is that it cannot establish incidence or causal inference beyond the diagnostic narrative.
