This case report describes a 51-year-old woman with known adenomyosis who was admitted with anemia with schistocytosis, thrombocytopenia, and acute renal failure, with thrombotic microangiopathy (TMA) initially considered. Plasma exchange and steroid therapy improved laboratory parameters, but renal biopsy showed acute tubular necrosis (ATN) without evidence of TMA, and thrombocytopenia was attributed to disseminated intravascular coagulation (DIC) rather than TMA. The paper notes that although ARF in adenomyosis is usually related to obstructive uropathy, this rare presentation links hemolytic anemia, DIC, and ARF due to ATN to adenomyosis. This paper is centrally about adenomyosis — it reports a rare case where adenomyosis is associated with hemolytic anemia, DIC, and acute renal failure from ATN.

Abstract

A 51-year-old woman with adenomyosis was admitted because of anemia with schistocytosis, thrombocytopenia, and acute renal failure (ARF). Thrombotic microangiopathy (TMA) was considered. Plasma exchange and steroid therapies improved laboratory results. However, renal biopsy specimen revealed acute tubular necrosis (ATN), but not TMA, and thrombocytopenia, diagnosed it as disseminated intravascular coagulation (DIC) but not TMA. Few cases of DIC associated with benign tumors of the uterus and, especially, adenomyosis have been reported. In adenomyosis patients, ARF is usually caused by obstructive uropathy. However, the rare case suggests that hemolytic anemia, DIC, and ARF due to ATN can occur in adenomyosis patients.
