Endometrial Stromal Sarcoma Arising in Colorectal Endometriosis: A Case Report and Review of the Literature

This paper reports a 40-year-old woman with low-grade endometrial stromal sarcoma (ESS) arising in colorectal endometriosis, identified after evaluation of change in bowel habits and rectal bleeding; diagnosis was pursued using imaging, colonoscopy/endoscopic biopsy, intraoperative frozen section, and immunohistochemistry, with CD10 and estrogen/progesterone receptor positivity and CD117/CD34 negativity. Key findings were disseminated intra-abdominal disease with poor prognosis, as the tumor was unresectable due to metastases and adhesions and the patient declined further adjuvant therapy, dying 18 months after diagnosis; the authors explicitly note limitations in evidence availability because ESS is rare and they could not determine evidence-based treatment guidance.
