Immunohistochemical Analysis of the Effect of Dienogest on Ovarian Endometriotic Cysts

The study evaluated the effect of dienogest on ovarian endometriotic cysts using immunohistochemistry in cyst specimens from 12 treated patients and 20 hormone-naive controls, assessing ER-α, progesterone receptor A (PRA), and Ki-67 proliferation indices in epithelial and stromal cells. Dienogest significantly reduced the cell proliferation index in both epithelial and stromal cells compared with controls, while ER-α and PRA expression showed no significant differences between treated and untreated cysts; ER-α and PRA were described as low in epithelial cells but well preserved in stromal cells regardless of treatment. In controls, Ki-67, ER-α, and PRA did not differ between proliferative and secretory phases. The authors note that reduced proliferation without changes in hormonal receptor expression may not support a direct progestational effect of dienogest on ovarian endometriotic cysts.
