Update on the treatment of endometriosis

This review summarizes up-to-date evidence on treatment of endometriosis, focusing on outcomes of laparoscopic surgery for pelvic pain and infertility, and efficacy of newer medical therapies such as aromatase inhibitors, anti–tumor necrosis factor-α agents, and dienogest, largely using recent Cochrane reviews and clinical reports. For pelvic pain, the review reports that non-comparative surgery studies usually show symptom improvement but also substantial pain recurrence or re-operation rates (reported 24%–54%), with higher repetitive surgery risk in women under 30 years and in those with pelvic pain. Controlled and meta-analytic data cited suggest laparoscopic surgery can have an advantage over diagnostic laparoscopy only for pain at 6 and 12 months, and a Cochrane review on ovarian endometrioma indicates excisional surgery is associated with reduced recurrence for several pain outcomes compared with ablative surgery, while evidence about fertility after ovarian stimulation and IUI is insufficient. This paper is centrally about endometriosis — it reviews and synthesizes current evidence on surgical and medical treatment options and outcomes.
