Deep Endometriosis Induced Spontaneous Colon Rectal Perforation in Pregnancy: Laparoscopy Is Advanced Tool to Confirm Diagnosis

This paper reports a case of a 32-year-old woman at 25 weeks’ gestation who presented with acute diffuse abdominal and pelvic pain and was taken for emergent exploration for presumed diffuse peritonitis; explorative laparoscopy identified diffuse severe purulent peritonitis originating from a small (<1 cm) double sigmoid and rectal perforation linked to an endometriotic nodule, confirmed by histopathology. The authors describe proposed mechanisms involving adherence and traction of the enlarged uterus on a strictly adherent sigmoid/rectosigmoid segment, and they note pregnancy-related decidualization of endometriotic nodules as a possible contributor to weakening of the intestinal wall. A limitation is that, beyond this single case and its literature context, no broader causal assessment is possible and risk factors were absent in this patient.
