This paper describes a 33-year-old woman who developed acute abdominal pain, distension, tachycardia, and peritonitis three days after an elective lower segment Caesarean section at 37 weeks, with a history of terminal ileal/caecal Crohn’s disease that was controlled and untreated during pregnancy. High-level evaluation included urgent CT showing free intraperitoneal air/fluid and hyperdense material in the pouch of Douglas, followed by emergency laparotomy that found a 1 cm caecal perforation with thickened caecal tissue and an intact Caesarean suture line; the perforation was excised/over-sewn with an omental patch and recovery was uneventful. Histology of the excised caecum showed decidualised endometriosis infiltrating the bowel wall through to the mucosa with ulceration, and there was no evidence of Crohn’s disease, and the authors note bowel perforation from endometriosis is rare, with only a small number of reported pregnancy/postpartum cases.
