Role of laparoscopy in evaluation of chronic pelvic pain
Diagnostic laparoscopy detected significant pelvic pathology in 66% of women with chronic pelvic pain, significantly outperforming clinical examination in identifying the etiology.

This study reviewed 86 women hospitalized for chronic pelvic pain lasting at least 6 months who underwent diagnostic laparoscopy under general anesthesia between 1999 and 2003, correlating preoperative pelvic examination findings with laparoscopic findings and re-evaluating treatment strategy. While pelvic exam abnormalities were detected in 33 (38%) women, laparoscopy found significant pathology in 57 (66%), with substantial diagnostic “error” in pelvic examination (21–58%) and a notable proportion of discrepancies in both directions. The most common laparoscopic findings were pelvic adhesions (20.9%) and pelvic congestion (18.6%); endometriosis was observed in 4.7% of patients, with mostly stage I disease, and some other conditions such as small subserous fibroids were missed preoperatively. A key limitation stated is that for adhesiolysis, only 33% of the subset undergoing adhesiolysis seemed to benefit after 1 year, and the small number warrants larger prospective studies, and pelvic congestion diagnosis was based on laparoscopist expertise without prior venography. This paper is centrally about endometriosis in chronic pelvic pain evaluation—specifically reporting a low observed incidence of endometriosis (4.7%) found at laparoscopy in women with chronic pelvic pain.
