Abnormal uterine bleeding in perimenopausal women: Correlation with sonographic findings and histopathological examination of hysterectomy specimens

This tertiary-hospital study evaluated 103 perimenopausal women (age 40 to within 1 year of menopause) with abnormal uterine bleeding who underwent hysterectomy, comparing transvaginal ultrasonography (USG) findings with histopathology of endometrium, myometrium, and cervix to assess diagnostic correlation. USG suggested fibroid uterus in 45.63% of cases, bulky uterus in 29.12%, adenomyosis in 10.69%, and thickened endometrium in 11.65%, while histopathology confirmed myometrial fibroid in 44.66% and adenomyosis in 20.39%, with endometrial hyperplasia predominating on HPE (56.31%), and USG thickness in postmenopausal patients correlating with abnormal endometrial pathology. The authors report that USG specificity for adenomyosis was high but sensitivity was low (47.62%), explicitly noting USG limitations in tissue characterization for adenomyosis and that USG alone was not sufficient for endometrial hyperplasia diagnosis.
