This study assessed endometrial perfusion using transvaginal color Doppler ultrasound by measuring blood flow waveforms in right and left uterine arteries in infertile women across 60 spontaneous menstrual cycles and 37 clomiphene citrate–stimulated cycles, followed from the follicular through the luteal phase. Uterine arterial blood flow increased significantly from the follicular phase to ovulation in spontaneous cycles and then rose markedly to about 200–230% of follicular levels after the 6th day of ovulation, whereas in clomiphene-stimulated cycles flow showed no change during the periovulatory period and increased to about 180–220% after the 6th day. The authors concluded that clomiphene stimulated cycles have lower periovulatory endometrial perfusion than spontaneous cycles, and they stated that Doppler assessment could help reveal unexplained infertility problems in induced ovarian cycles.
