After six months, she returned with recurrent symptoms and imaging that showed a left kidney hydroureteronephrosis with ureteral involvement, and cystoscopy with attempted ureteral catheterization could not pass beyond 2–3 cm from the orifice; she declined nephrostomy. The second operation and histopathology identified endometriosis ovarii sinistri, and the authors emphasize that urinary tract endometriosis can be clinically silent and that CT of the pelvis supports accurate diagnosis of this rare condition, while noting the overall diagnostic difficulty and lack of consensus on best diagnostic technique.
