Rectovaginal endometriosis: head to head comparison between sonovaginography and magnetic resonance imaging
Sonovaginography demonstrated higher sensitivity and comparable specificity to MRI in diagnosing rectovaginal endometriosis in surgically confirmed cases.

This retrospective study compared diagnostic accuracy of sonovaginography versus magnetic resonance imaging (MRI) for rectovaginal endometriosis (RVE) in consecutive women referred with suspected RVE, using surgical excision with histology as the confirmation standard. Among 189 referrals, 60 had RVE confirmed, and sonovaginography showed higher sensitivity than MRI (95% vs 81.7%) with similar specificity (93.8% vs 91.2%), and all surgeries were completed without complications with minimal discomfort during procedures. The main caveat explicitly emphasized is that sonovaginography is operator-dependent, and MRI’s performance may also vary by experience, with MRI described as typically needing less operator expertise.
