Abstract

症例は36歳，女性．右鼡径部に圧痛を伴う弾性軟の３cm大の腫瘤を認め，当科紹介となった．腫瘤は腹圧で変化なく，圧迫にても還納されず，腹部単純CTでは囊胞状を示した．外鼡径ヘルニアおよびNuck管水腫の診断で手術を行った．手術は前方アプローチで行い，水腫を破損や遺残のないよう完全摘出し，鼠径ヘルニアはMarcy法で修復した．病理学的所見では，摘出された多房性囊胞は中皮由来で，上皮直下に内膜間質様細胞を認め，子宮内膜症の組織像を伴っていたことから子宮内膜症を伴ったNuck管水腫と診断した．成人女性のNuck管水腫では，稀に子宮内膜症を伴うことがあり，ヘルニアの根治性に加えて遺残なく水腫を摘出することが重要と思われた．
