Abstract

子宮内膜症は, 子宮に主病変の存在する内性子宮内膜症と子宮以外の部分に子宮内膜組織が発育する外性子宮内膜症に分類される。中でも肝に発生する外性子宮内膜症は非常に稀である。今回われわれは, 肝嚢胞と術前診断した肝臓外性子宮内膜症の1例を経験したので報告する。症例は26歳, 女性。心窩部痛を主訴に受診。画像診断にて, 肝左葉を中心とする16×10cmの多房性巨大嚢胞が認められた。巨大肝嚢胞の診断にて腹腔鏡下肝嚢胞開窓術を施行した。切除した嚢胞壁の病理組織学検査にて, 異所性の子宮内膜組織が認められ, 肝臓子宮内膜症と診断した。
