This chapter reviews normal female gonadal physiology from puberty through reproductive age to menopause, emphasizing how ovarian pathology such as endometriomas, polycystic ovary syndrome, teratomas, and benign tumors can disrupt ovarian function, potentially leading to anovulation and sterility. It outlines the interaction between ovarian pathology and endocrinology, with the main focus on laparoscopic ovarian-preserving surgical techniques, while also discussing alternative treatments and screening tests. A key limitation is that it is a narrative chapter rather than an original study, so it does not provide new empirical results or a specific, explicit evidence-grade methodology. Relevance to endometriosis: endometriomas are listed among the cystic ovarian pathologies discussed in relation to abnormal ovarian function and endocrine interaction, though the chapter’s main focus is laparoscopic ovarian-preserving surgery and ovarian physiology across reproductive age and menopause.

Abstract

In patients with cystic ovarian pathology, such as endometriomas, polycystic ovary syndrome (PCOS), teratomas or benign tumours, there is a tendency for abnormal ovarian function. Anovulation and sterility may occur. This chapter summarises normal physiology of the female gonad and the development of oogenesis throughout the reproductive age to menopause. Furthermore, the interaction of ovarian pathology and endocrinology is described. The main focus is on the technique of ovarian-preserving surgery by laparoscopy. In addition, alternative therapeutic approaches and screening tests are discussed.
