Diffusion-weighted imaging (DWI) of adenomyosis and fibroids of the uterus
This study evaluated differences in apparent diffusion coefficient (ADC) values between uterine fibroids and adenomyosis, finding statistically significant distinctions and less heterogeneity in adenomyosis.

This retrospective study compared diffusion-weighted MRI apparent diffusion coefficient (ADC) values between uterine fibroids and adenomyosis in 50 patients who underwent pelvic MR imaging, using b factors of 50, 400, and 800 s/mm on a 1.5 T scanner. ADC regions of interest were measured in fibroid tissue, adenomyosis areas, normal myometrium, skeletal muscle, and urine, and histogram analysis in subsets (20 cases each) assessed tissue heterogeneity. Adenomyosis and fibroids showed statistically significant ADC differences versus normal myometrium and versus each other (adenomyosis mean 0.86 ± 0.30 vs fibroids 0.64 ± 0.29 × 10−3 mm2/s), and histogram analysis indicated less heterogeneity in adenomyosis than in fibroids, with the main caveat being that ADC differentiation was studied retrospectively without explicit histopathologic confirmation details in the abstract. This paper is centrally about endometriosis and/or adenomyosis — specifically adenomyosis imaging differentiation from fibroids using quantitative DWI/ADC measurements.
