This paper reports a single 27-year-old woman with symptomatic deeply infiltrating rectovaginal endometriosis with bowel involvement who declined surgery and other endocrine therapies due to fear of complications and side-effect concerns. Instead, the patient used dienogest 2 mg daily via vaginal application for 3 months; the authors assessed symptoms, rectovaginal/colposcopic findings, ultrasonography, and hormone levels (including suppressed estradiol and progesterone). After treatment, vaginally visible disease and related symptoms entered remission with amenorrhea and markedly improved pain (VAS for examination decreased), while the rectal component showed no significant change in node size on palpation/ultrasound despite reduced pain and only ongoing contact bleeding. The major limitation is that this is an uncontrolled case report, so efficacy and changes in rectal nodule size cannot be generalized.
