A Detailed Study in Adenomyosis and Endometriosis: Evaluation of the Rate of Coexistence Between Uterine Adenomyosis and DIE According to Imaging and Histopathology Findings
This study evaluated the diagnostic accuracy of TVS and MRI for adenomyosis and found diffuse adenomyosis was more frequent without rectal/rectovaginal septum DIE, with no other relationship between adenomyosis subtypes and endometriosis lesions.

This retrospective cross-sectional study evaluated relationships between uterine adenomyosis (including focal and diffuse subtypes) and endometriotic lesions—ovarian endometrioma and posterior deep infiltrative endometriosis (DIE)—using data from 154 women undergoing laparoscopic surgery with adenomyosis and endometriosis established by imaging, intraoperative findings, or pathology. It also assessed the diagnostic performance of transvaginal ultrasonography (TVS) and MRI for adenomyosis, finding that TVS had higher sensitivity (72.1% accuracy; 77.6% sensitivity) whereas MRI was more specific, particularly for diffuse adenomyosis with simultaneous uterine leiomyoma (specificity 90.0%). For subtype associations, diffuse adenomyosis was more frequent when rectal and rectovaginal septum (RVS) DIE was absent, but no relationship overall was found between adenomyosis subtypes and endometriotic lesion types. Limitations include the retrospective design and restriction to women with coexistence of endometriosis and adenomyosis who proceeded to surgery, which may limit generalizability.
