The paper reports a rare Japanese case of sigmoid colon endometriosis causing ileus in a 29-year-old married woman with prior ovarian endometriosis-related surgery (left ovarian cystectomy) and a right tubal procedure. She developed abdominal pain and worsening symptoms, underwent creation of an artificial anus for ileus, and further evaluation with barium enema and colonoscopy showed a complete obstruction at about 20 cm from the anal verge; at laparotomy, intraoperative frozen-section pathology identified sigmoid colon endometriosis, leading to sigmoid colectomy. Postoperatively, she received hormonal therapy with danazol and was followed clinically.
