Abstract

子宮内膜症は子宮内膜が異所性に増殖したもので，鼠径部発生例は少なく，なかでも子宮内膜症の症状を欠くsilent typeは非常に頻度が低い．症例は32歳女性．左鼠径部に腫瘤を自覚したが無症状であった．超音波検査で囊胞性部分と充実性部分を持つ直径２cmの腫瘍を認め，周囲組織を含めて摘出した．病理組織所見で子宮内膜腺管を認め，免疫組織染色でエストロゲンレセプター，プロゲステロンレセプター，CD10陽性で，異所性子宮内膜症と診断した．発生機序について子宮内膜組織に由来する説と子宮内膜以外の組織に由来する説がある．前者は機械的移植説，血管やリンパ管経由の転移説，径卵管移植説がある．後者は胎生期組織遺残説や誘導説がある．CD10は急性リンパ芽球性白血病の特異的抗原である．近年，リンパ腫や血液癌に加えて，子宮内膜症診断でもその有用が確認されており，本症例の診断にも有用であった．
