Abstract

症例は31歳の未婚女性で，26歳時より子宮内膜症の既往があり手術歴はない．2006年7月上旬，臍周囲部痛，嘔吐で来院．癒着性腸閉塞の診断でイレウス管を挿入し保存的加療をした．症状は軽快したもののイレウス管造影検査で回腸に狭窄を認めた．診断も含めて腹腔鏡下手術を施行したところ，回腸末端から50cm口側の回腸が子宮前面に癒着し屈曲していた．癒着を剥離し，腸管自体には肥厚や狭窄を認めず，閉塞は解除されたため腸管切除術は施行しなかった．病理組織学的検査所見で癒着組織に子宮内膜組織を認め，回腸子宮内膜症と診断した．術後補助療法としてLH-RHアゴニストを投与し，術後2年半無再発である．本邦報告49例では腸閉塞をきたした回腸子宮内膜症に対して全例に腸管切除術を施行しており，腸管切除術を行わなかった症例報告はない．腹腔鏡下癒着剥離術と術後ホルモン療法で加療した症例を経験したので，文献的考察を加え報告する．
