Xanthogranulomatous Salpingitis Associated with a Large Uterine Leiomyoma
This case study describes xanthogranulomatous salpingitis in a 50-year-old woman with a large uterine leiomyoma, a presentation lacking a history of pelvic inflammatory disease or endometriosis.

This paper reports a case of xanthogranulomatous salpingitis, a rare destructive chronic inflammatory process, in a 50-year-old woman evaluated for abnormal uterine bleeding attributed to a bulky uterus with a large posterior submucosal leiomyoma. High-level methods included pelvic ultrasound, hysterectomy with left salpingo-oophorectomy, and histopathologic examination showing lipid-like, foamy histiocytes with mixed inflammatory infiltrates and chronic fibrosis in the left fallopian tube, with negative special stains for bacteria and fungi. The authors note key caveats that the patient lacked a history of PID, IUD use, or lipid metabolism disease, and although endometriosis was suspected intraoperatively due to adhesions, there was no histologic evidence—so “burnt out” endometriosis cannot be fully excluded. Relevance to endometriosis: the paper discusses endometriosis as a proposed predisposing factor for xanthogranulomatous salpingitis and specifically describes intraoperative suspicion for endometriosis despite the absence of histologic findings, though the main focus is this single case report of xanthogranulomatous salpingitis associated with a uterine leiomyoma.
