症例は34歳，女性．7ヵ月間に4度，月経期間中に気胸を繰り返したため，胸腔鏡下手術を行った．胸腔内を観察し，横隔膜腱中心に数ヵ所の瘻孔が認められたため，すべての瘻孔を含むように横隔膜部分切除を施行した．術後病理検査では，胸腔側の瘻孔周囲には異所性子宮内膜組織が認められなかったが，腹腔側から出血斑（blueberry spot）が観察され，異所性子宮内膜組織を認めた．術後9ヵ月後に右気胸を再発したため，ホルモン療法を開始し，以後は再発を認めていない．経胸腔アプローチによる手術療…
