A rare case of de novo gigantic ovarian abscess within an endometrioma.
This report details a rare case of a de novo gigantic ovarian abscess within an endometrioma, presenting as a surgical emergency in a nulliparous woman, requiring 13 liters of pus drainage and subsequent hysterectomy with bilateral salpingo-oophorectomy.

This case report describes a 47-year-old nulliparous woman who presented with fever, hypotension, and a rapidly enlarging, tense abdomen, and was found on CT to have a de novo gigantic right ovarian mass with massive ascites and left ureteric obstruction. At laparotomy, six liters of pus were aspirated from the abdomen before visualization of structures, and an additional five liters of thick pus were drained from a right ovarian cyst reaching the right hemi-diaphragm; cultures grew Bacteroides fragilis, and histology showed endometriosis in the ovarian cyst wall and surface, with endometriotic cysts on the contralateral ovary. The authors note limitations inherent to a single case and emphasize uncertainty about the pathway of infection, while also citing prior literature suggesting endometrioma increases risk of tubo-ovarian/ovarian abscess and that risk factors can include advanced endometriosis and nulliparity. This paper is centrally about endometriosis — it reports a gigantic spontaneous ovarian abscess arising within an endometrioma, confirmed by histology and contextualized with risk factors for abscess in endometriosis.
