Malignant Transformation of Endometrioma in a Woman with a History of Ovulation Induction and In Vitro Fertilization
This case report describes the successful surgical management of ovarian endometrioid adenocarcinoma arising within an endometrioma in a woman with a history of ovulation induction and IVF.

This paper reports a clinical case of a 33-year-old woman with PCOS who underwent ovulation induction with gonadotropins and IVF and later had a frozen embryo transfer cycle during which ultrasound detected a complex ovarian cyst with a papillary lesion and Doppler evidence of venous flow, concerning for malignancy despite a normal CA125 level. Exploratory laparotomy identified a left endometriotic cyst adherent to the rectosigmoid, which ruptured during surgery, and final pathology showed stage 1c, grade 2 intracystic endometrioid adenocarcinoma arising in an endometriotic cyst; after reoperation and adjuvant carboplatin/taxol chemotherapy, pathology showed no residual disease, and the authors note the challenge of distinguishing effects of fertility drugs from confounding factors like indication and surveillance in retrospective studies. The paper does not explicitly discuss endometriosis or adenomyosis; it was included in the corpus via a keyword match in the upstream search index.
