This long-term follow-up study evaluated reproductive outcomes, symptom persistence, and recurrence after a radical fertility-preserving laparoscopic approach for rectovaginal septum endometriosis with rectal involvement. Fifty-five premenopausal patients underwent combined laparoscopic-transvaginal en bloc resection of the Douglas pouch area with partial rectovaginal septum and partial rectal resection, followed by end-to-end anastomosis, and 30 patients returned questionnaires with a median postoperative interval of 94 months. Among respondents, 18 pregnancies were reported in 11 patients, and 27/30 had no complaints after surgery; 2 required reoperation for endometriosis recurrence and 1 reported increased symptoms.
