Abstract

症例は23歳,女性.怒責時の右鼠径部痛で受診された.右鼠径部に腹圧で膨隆する腫瘤を触知し圧痛を認めた.CTでは右鼠径部に嚢胞性腫瘤を認めた.右鼠径ヘルニアに併存した嚢胞と判断し手術を行った.鼠径管を開放し暗赤色の嚢胞とそれに続くヘルニア嚢を確認した.ヘルニア嚢を高位結紮して嚢胞ごと摘出しMarcy法で内鼠径輪を閉鎖した.病理組織学的所見では,嚢胞は中皮細胞により内腔を被覆された腹膜鞘状突起であり子宮内膜症の組織像を伴っていた.女性において,開存して残存した腹膜鞘状突起はNuck管と呼称され,そこに液体の貯留したものがNuck管水腫である.子宮内膜症は子宮内腔以外で子宮内膜細胞が増殖したもので,Nuck管水腫内での発生はまれである.今回われわれは,成人のNuck管水腫内に発生した子宮内膜症の症例を経験したので報告した.
