O artigo descreve um estudo de caso de uma paciente de 49 anos com evolução de endometriose profunda na pós-menopausa, com sintomas desde a adolescência (dispareunia, hipermenorreia e dor pélvica/abdominal) e diagnóstico na vida adulta. A paciente foi submetida a múltiplas abordagens cirúrgicas por endometriose, desenvolvendo menopausa prematura, e precisou de nova cirurgia após surgirem focos endometrióticos profundos; o mapeamento das regiões acometidas foi feito para planejamento, e a exérese de focos e aderências foi realizada por videolaparoscopia, com confirmação anatomopatológica. Alguns dias depois foi necessária reabordagem, culminando em iliotilectomia por focos intestinais endometrióticos, também confirmados no anatomopatológico, e a paciente segue sem queixas com “ótimo prognóstico”. Como limitação, trata-se de relato de caso, refletindo a baixa prevalência e escassez na literatura.
