Abstract

当院で経験した後縦隔ミュラー管囊胞の2切除例を報告する．【症例1】50歳，女性．腎腫瘤の精査中に後縦隔の囊胞性腫瘤を指摘された．MRIにてTh7椎体左側に30×20 mm大のT2強調で内部highな表面平滑な囊胞を認めた．神経原性腫瘍を疑い胸腔鏡下に切除した．病理組織診では，単層の線毛円柱上皮に覆われた薄い線維性の壁を有する囊胞性病変で，免疫染色では上皮はER，PgR陽性であり，ミュラー管囊胞と診断された．【症例2】36歳，女性．人間ドックの胸部Xpにて椎体左側，大動脈弓の頭側に腫瘤性病変を指摘された．胸部CTにてTh4椎体左側に接して30×18 mmの内部均一な囊胞性腫瘤を認めた．神経原性腫瘍を疑い胸腔鏡下に切除した．病理組織診では多列線毛円柱上皮に裏打ちされた囊胞性病変で気管支原性囊胞と診断された．症例1を経験後，免疫染色を追加し，上皮成分はER，PgR陽性であったことから診断を訂正した．
