Abstract

Laparoskopi Kistektomi tetap menjadi pilihan lini pertama untuk penanganan kista endometriosis. Namun tindakan ini akan menimbulkan kerusakan terhadap ovarium. Tindakan “fertility sparing” adalah dilakukannya manajemen terhadap pasien endometriosis dengan menghemat uterus dan ovarium sehingga tidak banyak jaringan yang terbuang. Tujuan Penelitian : Penelitian ini membandingkan nilai cadangan ovarium sebelum dan setelah tindakan laparoskopi kistektomi dengan memakai tiga marker yaitu kadar AMH (Anti Mullerian Hormon), kadar FSH dan Estradiol pada siklus haid hari kedua atau ketiga dan nilai AFC (Antral Follicle Count) dengan USG Trasnvaginal.  Materi dan metode : Penelitian ini adalah penelitian eksperimental yang dilakukan terhadap 25 orang penderita endometrioma. Cadangan ovarium sebelum dan setelah tindakan laparoskopi dinilai dengan tiga marker yaitu AMH, FSH dan Estradiol serta AFC. Dilakukan laparoskopi kistektomi dengan teknik yaitu tanpa energi elektrik koagulasi yang berlebihan, obat vassopresin yang disuntikkan ke dinding kista, teknik “stripping of the membrane” pada kista, tanpa suturing/ penjahitan, teknik kompresi untuk mengatasi perdarahan, menghindari pengangkatan kantong kista hingga ke daerah hilus. Analisis statistic untuk menilai kemaknaan menggunakan paired t test. Hasil : Terdapat perbedaan rerata kadar AMH dimana sebelum laparoskopi lebih tinggi yaitu 1,77 ± 0,39 ng/ml sedangkan setelah laparoskopi 1,54 ± 0,38 ng/ml. Rerata kadar FSH dimana setelah laparoskopi lebih tinggi yaitu 10,13 ± 6,51 mlU/ml sedangkan sebelum laparoskopi 6,91 ± 4,59 mlU/ml. Rerata kadar estradiol dimana sebelum laparoskopi lebih tinggi yaitu 99,65 ± 77,18 mlU/ml sedangkan setelah laparoskopi 63,67 ± 35,22 mlU/ml. Terdapat perbedaan AFC antara sebelum laparoskopi dan setelah laparoskopi dimana 15 orang (60%) AFC <4, 10 orang (40%) AFC 4-6, saat sebelum laparoskopi dan 8 orang (32%) AFC <4, 8 orang (32%) AFC 4-6, dan 9 orang (36%) AFC 7-10 saat setelah laparoskopi. Terdapat hubungan yang bermakna dari hasil analisis statistic ( terjadi penurunan cadangan ovarium sebelum dan setelah tindakan laparoskopi kistektomi “fertility sparing”) (p=0.001). Kesimpulan : Terdapat pengaruh tindakan laparoskopi kistektomi terhadap penurunan cadangan ovarium yaitu AMH mengalami 13,3% penurunan (p=0,001) , FSH mengalami 31,6% kenaikan (p=0,001), estradiol mengalami 47,8% penurunan (p=0,001), dan AFC mengalami 47,9% kenaikan (p=0,003).Kata kunci: Laparoskopi Kistektomi, Endometriosis, Anti Mullerian Hormone, Antral Follicle Count, FSH, Estradiol

Objective

was to compare the ovarian reserve before and after laparoscopic cystectomy “fertility sparing†using three marker of ovarian reserve which are AMH, FSH and Estradiol on 2nd or 3rd day of menstrual period and counting the AFC by Transvaginal Ultrasound.

Methods

This study is an experimental study on 25 endometrioma patient. Ovarian reserve values was taken before and after laparoscopy using three markers which are AMH, FSH and Estradiol and AFC. All patient included to group performed laparoscopy cyctectomy with several techniques which are no or less use of electrocoagulation, using vassopresin injected to the cyst wall, using stripping of the membrane technique on cyst, without suturing, using compression to control bleeding, avoiding hillus cut. Statistical Analysis was using paired t test method.

Result

There was average differences on AMH value before 1,77 ± 0,39 ng/ml and after laparoscopy 1,54 ± 0,38 ng/ml. FSH value before was 6,91 ± 4,59 mlU/ml and after was 10,13 ± 6,51 mlU/ml, for estradiol before was 99,65 ± 77,18 mlU/ml and after was 63,67 ± 35,22 mlU/ml. There were 15 samples before laparoscopy has AFC <4, 10 samples has AFC 4-6, after laparoscopiy there was 8 samples has 8 AFC, 8 samples has 4-6 AFC, and 9 samples has 7-10 AFC. There was a statistical significance on the reduce of ovarian reserve before and after laparoscopic cystectomy “fertility sparing†(p=0,001).

Conclusion

There was the affect of laparoscopy cystectomy to the decrease of ovarian reserve which sre 13% decreaed of AMH (p+0,001), 31,6% increased of FSH (p=0,001), estradiol was 47,8% decreaed (p=0,001) and AFC has increased 47.9% (p=0,003).
