This paper reports a single case of appendicular endometriosis (AE) in a 35-year-old woman with intermittent periumbilical and right iliac fossa pain for one year and a history of infertility, whose imaging was interpreted as suggesting acute appendicitis and who also had ovarian cysts. Ultrasound and contrast-enhanced CT led to planning for laparoscopic appendectomy, and intraoperatively a tumor-like mass involving the mesoappendix was excised; histopathology later showed endometrial glands and stroma without inflammation, and the patient was symptom-free at 3 months.
