Abstract

主リンパ節に内膜組織を認めた腸管子宮内膜症の稀な1例を報告する．症例は49歳の女性で，右下腹部痛を主訴に当院救急外来を受診．受診時，腹膜刺激症状を伴う右下腹部痛と炎症反応の著明な上昇を認めた．造影CTでは回盲部に4cm大の不均一な造影効果を伴う腫瘤形成と周囲リンパ節腫大を認めた．加えて，虫垂壁肥厚と周囲脂肪識濃度上昇および近傍腹壁に膿瘍形成を認めた．以上より，腫瘍による続発性穿孔性虫垂炎・膿瘍形成が疑われたために，緊急でD3郭清を含む回盲部切除術を施行した．病理組織学的に，虫垂，および回腸の粘膜下層から漿膜下組織にかけて子宮内膜腺に類似した腺管を認め，免疫染色から腸管子宮内膜症と診断した．また，虫垂所属リンパ節である#203にも同様に内膜組織を認めた．本邦において，主リンパ節に内膜組織を認めた腸管子宮内膜症の報告例はなく，非常に稀な1例を経験したので文献的考察を加えて報告する．
