This case report describes a 50-year-old woman of non-reproductive age presenting with a month of constipation and abdominal cramps that progressed to complete recto-sigmoid obstruction, with vomiting, mild fever, tachycardia, and inflammatory bloodwork. High-level evaluation included abdominal X-ray/CT showing colonic distension and sigmoidoscopy revealing a constricting lesion 11 cm from the anus that could not be bypassed; biopsies showed only mild nonspecific inflammation and no malignancy. Urgent laparotomy found a 5 × 6 cm obstructing mass at the recto-sigmoidal junction with adhesions to the posterior uterus surface but no gross endometriosis, and histology of the resected specimen confirmed colonic endometriosis composed of benign endometrial glands and specialized stroma with acute inflammation. The paper is limited to a single case and notes that definitive diagnosis requires histology, with clinical presentation being atypical and diagnosis challenging when the patient proceeds directly to surgery.
